CCS is designed to provide services to individuals diagnosed with a mental health/serious mental illness/co-occurring disorders who are 18 years of age or older in need of structured short-term behavioral health services with 24-hour monitoring
Program Goals
• To improve adaptive functioning
• To elevate self-esteem, build self-reliance, and empower individuals to reach their full potential mentally, emotionally, and socially
• To rehabilitate and reintegrate adults experiencing serious mental illness, medical issues/substance abuse into the community
• To prevent psychiatric hospitalization
• To decrease the use of physical care services and the need of personal care
Admission Criteria
• The individual presents with psychiatric symptoms consistent with a DSM-IVR diagnosis which requires and can reasonably be expected to respond to therapeutic intervention, including substance abuse/dependence
• There are significant symptoms that interfere with the individual’s ability to function in at least one life area
• There is an expectation that the individual has the capacity to make significant progress toward treatment goals or treatment is necessary to maintain the current level of functioning
• Support network is adequate to ensure the individual’s safety and meets the needs, and/or support network is willing to participate in the individual’s therapy, complies with treatment recommendations, and does not interfere with the treatment process.
• Physical health services needs can be addressed in the scope of practice of the facility
• Personal care needs can be addressed in the scope of practice of the facility by coordination of care for physical health services
Admission Process
Intake
Individuals are referred for services by the T/RBHA clinical team. Eligibility will be determined through mutual agreement between the referral source, individual and TBHC administrator, will advise the clinical team and the individual of the requirements that must be met to complete the referral process.
Administrators determine the suitability of the referral for services by reviewing all medical, psychological, vocational, psychiatric, and social evaluation materials. Staff members are available for consultation with the clinical team and others involved for providing continuity of care.
Admissions/Re-Admissions
Upon referral, the Facility will conduct a face-to-face, individualized assessment of the individual. The Facility accepts for admission and re-admission and to its program for individuals who meet the following criteria:
-The individual presents with psychiatric symptoms and/or substance abuse/dependence consistent with a DSM-V diagnosis and can reasonably be expected to respond to therapeutic intervention
-Individual may require physical health services in which personal care needs can be addressed under the scope of practice of the facility
-There are significant symptoms that interfere with the individual’s ability to function in at least one life area
-There is an expectation that the individual has the capacity to make significant progress toward treatment goals or treatment necessary to maintain the current level of function
-Support network is adequate to ensure individual’s safety and needs are met, and/or support network is willing to participate in the individual’s therapy, complies with treatment recommendations, and not interfere with the treatment process
-Cases are managed by a Regional/Tribal Behavioral Health Authority in Arizona,
-Voluntarily agrees to participate in Facility programs
-Exhibits the ability to function safely within the program structure and environment
-Placement based on first come, first serve basis with consideration given to the severity of individual needs, and based on the availability of services
