Counseling includes groups or individual sessions according to the individual’s needs and services plan. Counseling will address difficulties associated with having a mental illness/substance abuse to improve familial relationships, social and personal functioning, substance use, crisis situations, physical and personal health, and to support the overall treatment and rehabilitative process. The amount of counseling provided will depend on the specific needs of the individual. A behavioral health professional is present at COLL COMMUNITY SERVICES to offer individual counseling services at minimum once a month. Family group sessions are offered every quarter to encourage family involvement with treatment and discharge planning.
The Facility provides assistance in self-administration of medication which includes:
-Storage of the individual’s medication
-Facilitate reminders when it is time to take medications
-Verifying that the medication is taken as directed by the individual’s medical practitioner by:
-Confirming that a medication is being taken by the individual for whom it is prescribed
-Checking the dosage against the label on the container
c. Confirming that the individual is taking the medication as directed
-Opening of the medication container for the individual
-Observing the individual while the individual removes the medication from the container or takes the medication
-A staff member inspects the storage and preparation of the individual’s medications at least once every three months to ensure compliance and documents the results of this inspection include:
-Name of staff member conducting inspection
-Date of inspection
-Area inspected
-Medication is stored according to requirements
-Medication is disposed of according to requirements
-Any action taken to ensure compliance with the requirements
Daily activities may include and are not limited to community meetings, educational, informative, and interactive groups, physical/wellness groups and activities lasting less than 4 hours.
Orientation/Training prior to providing services include:
• Resident rights
• Facility policies and procedures necessary for the performance of the staff member’s duties
• The staff member’s job description
• The Facility’s evacuation path
• Procedures for responding to a fire, a disaster, a hazard, a medical emergency and a resident experiencing a crisis situation
• On-line Relias Learning Center trainings Video
• Any other trainings required on employees Training Plan on or before due date
-Staff may require other trainings based on their job description including: a. First Aid
-CPR
-Assistance in Self-Administration of Medication
-BHT receive at minimum 24 hours of training during the first 12 months of full-time employment or the equivalent amount for part-time employment after the staff member’s starting date of employment or contracted services which may include time spent orientation and in acquiring the skills and knowledge mentioned above
-Subsequently, every 12 months of employment or contract service or the equivalent amount for part-time employment or contract service after the staff member’s first 12 months of employment or contract service receives at least 12 hours of training.
-During the first 12 months of employment of contract services training include the skills
COLL COMMUNITY SERVICES (CCS) will provide transportation to residents for appointments, treatment programs, socialization activities and other outings. We will use our own transportation to transport our client.
POLICY: In transport, the resident means that he or she will return to the behavioral health facility. In transfer, the resident is not returning to the behavioral health facility.
Transport policy: Except for a transport of a resident due to an emergency or transportation with an outside entity arranged by the resident or the resident’s representative, the administrator of designee shall ensure that a caregiver coordinates the transport and the services provided to the resident according to procedures below.

Outing will be planned with each resident's participation to ensure that resident preferences are taken into consideration. Any out of home activity lasting 4 or more hours will be designated as an outing. The administrator or designee will review each resident's medical information before scheduling an outing to ensure that:
-An outing is consistent with the physical ability, medical condition, and treatment needs of each resident participating in the outing; and
-Probable hazards, such as weather conditions, adverse resident behavior, or medical situations that may occur during the outing are identified and personnel members participating in the outing are prepared and have the supplies necessary to prevent or respond to each probable hazard.
The administrator or designee shall:
-Assign at least two personnel members for the intended outing to ensure each resident's health, safety, and welfare;
-Assign additional personnel members present for an outing as deemed necessary;
-Review records of personnel members participating in the intended outing to ensure that each personnel member on the outing has documentation of current training in CPR and first aid.
-Develop documentation before an outing that includes:
-The name of each resident participating in the outing;
-A description of the outing;
-The date of the outing;
-The anticipated departure and return times;
-The name, address, and, if available, telephone number of the outing destination; and
-The license plate number of each vehicle used to transport a resident;
-Update documentation to include the actual departure and return times and is maintained on the premises for at least 12 months after the date of the outing;
-Emergency information for each resident participating in the outing is maintained in the vehicle used to transport the resident and includes:
-The resident’s name;
-Medication information, including the name, dosage, route of administration, and directions for each medication needed by the resident during the anticipated duration of the outing;
-The resident's allergies; and
-The name and telephone number of the individual to notify in case of medical emergency or another emergency;
-The name and telephone number of the resident's case manager
-The name and telephone number of the resident’s physician
-A copy of the policy and procedure for outings is to be maintained in each vehicle used on the outing; and Personnel member(s) participating in the outing will document the safe return of each resident participating in the outing. The documentation will be recorded after arrival and will be in each participating resident's medical record under "Progress Notes".
Outside activities including work, shopping and worship services will be in accordance with the resident court orders and/or treatment plan and every effort will be made to accommodate the resident specific preferences. Schedule of such activities with the home may be required on a case-by-case basis and will be part of the resident progress medical record.
The resident's preferences in these areas will be recorded on the Admission Data form.
CCS will provide social, recreational and rehabilitative services each day to fill the hours that residents are not involved in other planned activities. These activities will be planned at least seven days in advance.
The schedule of activities will be posted conspicuously and any substitutions to an activity will be noted accordingly. The activity plan calendar will be kept on file for at least six months after the last date on the calendar.
The administrator shall review the weekly activities to ensure that adequate personnel are scheduled for any out of home activity(s).
